• A statement, made under penalty of perjury, that the notice is accurate and that you are authorized to act.
• Your physical or electronic signature.

We may forward a notice to the affected publisher or creator. Knowingly submitting a materially false notice may create liability. This channel also accepts direct creator takedown requests without requiring formal DMCA language.
